Soccer 4/1/13 (9:40 am) Sex: M S: A 19 y/o male soccer player complains of temporary head pain after his head collided with another players while going for a header. Athlete said that pain left shortly after and felt no more pain shortly after. Athlete stated everything was fine and he was ready to resume play. O: On Field Evaluation: Upon arrival athlete was alert and conscious. All palpations were normal with no pain. Got athlete to feet with no onset of symptoms and escorted him off the field. Off Field Evaluation: Cognitive/memory and cranial nerve tests were within normal limits. Balance tests were within normal limits. Motor tests were within normal limits. Upon exercise exertion (running in straight line) headache returned. Sat athlete down and will monitor athlete the rest of the game and refer to be seen by physician after the game. A: Working Dx: concussion. Problem List: headache with exercise exertion. STG: refer to physician. LTG: return athlete to symptom free play. P: Have athlete seen by physician. Application To CIP: This injury assessment demonstrated how safety measures were taken with the presence of a potential serious injury. Even though the athlete "felt fine" and ultimately the injury was not an emergency, proper actions were still taken to make sure of that. These precautions need to be taken to rule out the potential for a serious injury that could put the athletes well being at risk. Had it been necessary to use PPE due to blood, gloves and gauze would be present on the attending AT at all times. In the event that the athlete needed to be transported, the EAP would have been initiated with the proper steps being taken to remove the athlete properly in a respectable amount of time. |
